All clinic visits are scheduled by appointment only. The initial evaluation session lasts between 90 to 120 minutes and is an opportunity for Dr. Mu to review your history and discuss the relevant issues for which you are seeking care. Prior to this appointment it is advised that you download and complete the new patient information form and the new patient intake questionaire found under the resources tab. In addition there are 3 documents relating to office policy, release of information and consent for treatment. Please download and review these documents. Completed documents can be faxed or mailed to the office or you may choose to bring it with you to the first appointment. An optimal evaluation includes the input of a trusted informant, such as a parent, spouse, other family member or close friend; the presence of one such informant is encouraged, but is not required.
Followup appointments are scheduled as either 25 minute or 55 minute sessions.
For issues arising between office appointments, Dr. Mu is available by phone for brief phone sessions. There is no additional charge for such sessions. All attempts will be made to return your calls promptly. If there is an emergent issue for which you feel your safety is at risk, please call 911 or proceed to the nearest emergency room.
Each appointment time is reserved exclusively for a specific patient. If you need to cancel or reschedule an appointment, you are asked to give 24 hours' advanced notice. If you do not make the appointment and do not give the required notice, the full appointment fee will be charged.
